We are currently seeking a skilled and compassionate Speech Language Therapist to join our team.  Home Health therapy offers the perfect work/life balance through flexible scheduling.  Our Care Coordination team provides our therapist with valuable resources such as scheduling and documentation support so that you are able to focus on what matters most, patient care!!!

Key Responsibilities:

• Conduct thorough patient evaluations and assessments.


• Develop personalized treatment plans based on individual needs and goals.


• Implement therapeutic interventions and monitor progress.


• Educate patients and their families on treatment protocols and wellness strategies.


• Collaborate and facilitate with other healthcare professionals to ensure coordinated care.


• Maintain accurate patient records and documentation.


• Attend meetings and case conferences


• Communicating daily with office staff to accept new patients, complete paperwork and turn in payroll


• Completion of Evaluation, Plan of Care, Reassessment, and Discharge visits and paperwork.

Qualifications:

• Current Speech Language Pathologist Licensure


• Ability to travel to patient’s home or facility


• Strong communication skills


• Home healthcare experience preferred


• This position requires daily commute to patients’ homes and facilities in a 40–mile radius of Medina, OH
